Former clients may have a need to send records to new providers or otherwise request their medical records from Kerry Heath.  Kerry Heath retains medical records in accordance with federal and state laws.  The following are time frames for retaining records.  


If you were 18 or older at the time of discharge from Hope Counseling, your medical records will be retained for 6 years from the date of your last session.


If you were under the age of 18 at the time of discharge from Hope Counseling, your medical records will be retained until 1) 3 years beyond your 18th birthday, or 2) 6 years beyond the date of your last session, whichever is latest.  

Adults must make records requests for themselves.  Kerry Heath cannot respond to requests made by parents or spouses of adults due to confidentiality requirements even if the person was a minor at the time of service. Mrs. Heath will not respond to any such request unless the client (now an adult) is the one making the request.  The most efficient method of transferring records is to have your current provider send a signed records request to Kerry Heath via:

The above fax and email are both HIPAA compliant providers for added security.  Please allow a reasonable amount of time for Mrs. Heath to receive and respond to medical records requests.
